Job Description:
Position Summary: The Ambulatory Medication Safety Officer (MSO) is a part of the system pharmacy medication use advancement department team. This clinician works directly with providers in University Hospitals Health System including physicians, advanced practice providers, nurses, and pharmacists. This position is responsible for facilitating and assisting in the design of safe medication use systems for patients in ambulatory locations (e.g. UH retail pharmacies, and physician offices). Job Duties as assigned including but not limited to:
  • Support the system Medication Safety Program at UH.
  • Develops and monitors process for ambulatory and retail pharmacy safe medication use and compliance oversight.
  • Monitors, reviews, and synthesizes recommendations from reported adverse drug events and trends.
  • Participates in LFD/RCA, FMEA, and quality reporting activities.
  • Participates in the review of relevant technologies and processes.
  • Participates in the system UH formulary and policy process.
  • Identify areas of potential safety risks and lead medication use quality improvement projects.
  • Serve as an informational and patient problem solving resource for clinical staff pharmacists.
  • Serve as medication safety rotation preceptor to PGY1 and PGY2 pharmacy residents.
  • Chair Ambulatory Medication Safety Subcommittee; participate in other quality and safety meetings as necessary.

  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications:Education
  • Pharm.D (Required) or
  • Master's Degree Of Science in pharmacy (Required)
  • PGY2 Residency or applicable fellowship (Preferred)
Work Experience
  • 5+ years Of hospital pharmacy experience (Required) and
  • Performance improvement and regulatory experience (Preferred)
Knowledge, Skills, & Abilities
  • Computer systems skills in spreadsheet, database and word processing (Preferred proficiency)
  • Must demonstrate highly effective written and oral communication skills (Required proficiency)
  • PGY-1 and PGY-2 residency (drug information/medication safety) (Preferred proficiency)
Licenses and Certifications
  • Registered Pharmacist (R.Ph) in the State of Ohio (Required)
