: Job Details :

Plan, coordinate, implement, bill and monitor case management services for HIV positive clients through the Alachua County Health Department for Area 3/13. Perform Medical Case Management functions for Ryan White (RW) as specified by current rules and regulations from the State of Florida, Medicaid and any other governing agency. Document performed medical case management functions in Care. Ware, such as writing progress notes, Comprehensive Needs Assessment, Comprehensive Care Plan and Acuity. Comprehensive Needs Assessments need to be completed on an annual basis or as needed. Comprehensive Care Plans are to be updated every 6 months with client involvement. Acuity scale assessment is to be completed on an annual basis or as needed. Paper charts are to contain service authorizations, dental treatment plan, signed release of information, change of information form, lab results, medical records and any correspondence concerning the clients care.
