Polaris Healthcare and Rehabilitation Center is a beautiful 100 bed skilled nursing facility located at Milford Wellness Village. We are currently seeking an experienced Infection Prevention/Wound Care Nurse to join our nursing team!

This position is responsible for policy and program development, standards interpretation and regulatory analysis, epidemiology and outbreak analysis. Leadership of and/or participation in work groups or committees as required for IP program success. Employee Health related to employee infections/immunizations/prophylaxis. Active participation in environmental inspections, continuing education and in-services. Collaborates and partners with Staff Development to facilitate and promote performance improvement in Infection Control. Plays an active role during surveys and inspections by regulatory agencies. Maintains entries into DelVax system and completes all daily and weekly reporting requirements.
Position Responsibilities for the Infection Prevention/Wound Care Nurse:
- Follow new admissions and current residents on proper documentation of PPD, FLU, pneumonia and Covid vaccinations.
- Flu clinics for residents and staff
- Provide education on the Covid vaccinations and assist with facility in-services
- Manage the antibiotic stewardship documentation along with a monthly antibiotic line list
- Keep abreast of any new CDC warnings or regulations to COVID or any other potential diseases.
- On call rotation
- Unit rounding to ensure that all infection control practices are being maintained
- Rounding daily to ensure residents who need any type of isolation that proper signage is up and proper PPE is in place
- Maintain and update the facilities isolation list
Qualifications: for the Infection Prevention/Wound Care Nurse:
- Must be a nurse with an active License, preferably issued by the state of Delaware.
- Willingness to attain an Infection Preventionist certification.
- Three years experience providing direct care to residents in a long term care setting. Understands the principles of wound prevention and treatment.
- Familiarity with PCC and an ability to learn and independently manage specialized infection control software.
- An ability to work well with people in order to facilitate changes that prevent or control infection is required.
- Weekly wound rounds with our wound NP
- Weekly documentation of residents wounds along with MD and family notification
- Ensure that wound care is being done accurately and according to MD order
- Weekly oversight of the treatment cart to ensure proper supplies are available and that there are no outdated supplies
