POSITION SUMMARY:The Infectious Disease NP at the Addabbo Health Center is responsible to the Medical Director. Candidate is responsible for providing comprehensive health care and preventive services for high-risk patients such as HIV/AIDS patients. Diagnoses and treats diseases and injuries of human internal organ systems. Examines patient for symptoms of organic or congenital disorders and determines nature and extent of injury or disorder, referring to diagnostic images and test, and using medical instruments and equipment. Discusses medication and recommends dietary and activity program, as indicated by diagnosis. Refers patient to medical specialist when indicated. candidate is also responsible for assuring continuity of care, including patients admitted to the Addabbo's affiliated hospitals. The Infectious Disease NP will be an attending staff member with admitting privileges at the affiliated hospital, and may participate in staff conferences and specialty clinic sessions, as the Medical Director may provide. The Infectious Disease NP will provide hospital in-patient service coverage, as required by the affiliated hospital, the length of service determined by the Department Chairman of the affiliated hospital and the Medical Director of Addabbo.RESPONSIBILITIES:1. Evaluation, diagnosis and treatment for patients with infectious disease.2. Preventive care, consisting of periodic physical examination, immunizations, health education and preventive screening.3. Treatment, counseling, and referral for rehabilitation for maintenance of patients with chronic conditions.4. Coordination of specialty services for consultation and treatments.5. Referral and follow-up for patients requiring in-patient care at St. John's Episcopal Hospital/SSD, Queens Hospital Center, or others as required.6. Will be required to be on telephone on-call coverage, for patients.7. The Infectious Disease Physician will work cooperatively with other allied health providers including the nurses, medical team assistants, counselors, social workers, psychologist, psychiatrist, pharmacist, nutritionist and laboratory technicians in providing comprehensive team health care.8. The Infectious Disease Physician is expected to participate in Continuing Professional Education programs to provide 50 CME credits per year.9. The Infectious Disease Physician will be required to participate in other Health Center related tasks as directed by the Medical Director. These may include but not limited to Quality Assurance, Patient Education in the Community, Completion of Survey, etc.10.
