Job Description:
OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, save lives for our consumers, we hope to allow them to stay as active and engaged in their lives and in their own communities for as long as possible.To that end we are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.What you will do:
  • Initial and ongoing assessment of health and functional status of our members.
  • Telephone and in person assessments.
  • Develop community-based care plans and related service packages.
  • Participate in the development of a member's Individual Care Plan (ICP)
  • Arrange, coordinate, and monitor the provision of appropriate community services.
  • Promote the independent functioning of members.
  • Participate in the determination of a member's need for institutional and community long term care services.
What you will bring:
  • Your passion for helping others and desire to make a difference.
  • Bachelor's degree in social work or related field
  • Two years' experience in the care of persons 65 and older.
  • Working knowledge of the social, emotional, and physical processes of aging
  • Ability to gain and hold the confidence of clients and their families.
  • Knowledge and skill necessary to work successfully within a team; ability to express clearly, both orally and in writing
  • Ability to travel and reliable transportation.
