Position Inpatient Care Coordinator- BCDC Location Beaufort Memorial Hospital Full-Time/Part-Time Full-Time Shift Days Exempt/Non-Exempt Non-Exempt DescriptionReporting directly to the Behavioral Health Service Line Director, the Care Coordinator is responsible for providing comprehensive direct patient care coordination, case management services, coordination of and referral to health and social welfare resources and discharge planning to facilitate achievement of quality cost-effective outcomes. They will function as an integral member of a collaborative and interdisciplinary team, using assessments, implementing, coordinating, monitoring and evaluation skills. The Care Coordination Department supports Clinical and non-clinical ancillary services in assuring the continuum of care for patients while serving as patient advocate in appropriate utilization of benefits and community resources.Position Requirements Education:
- Associate's degree or license in Social Work, Counseling, Nursing (RN), or similar field required. Bachelor's or Master's degree preferred.
Experience:
- At least one year of experience in a healthcare setting and/or acute care; and/or case management experience required.
- Minimum one year of experience in Behavioral/Mental Health for positions in Behavioral Health areas.
Certifications/Licenses:
- Must maintain current SC license in field of expertise (if applicable)
