SummaryUnder the direct supervision of the Director of Care Management and the Lead Social Worker II, The Social Worker I is consulted to provide assessment and referral for services related to the complex care needs of the patient and family for adjustment to illness, crisis intervention and to preserve patient's self determination in the decision making process.Education/Experience3 to 5 years of Clinical Social Work experience required. Acute Care Social Work experience preferred. Must have either Bachelor's degree in Social Work or Master's degree in social work.Certification, Registration & LicensureNone required.Responsibilities* Conducts a comprehensive assessment of the patients/families behavioral, psychological and socioeconomic needs. Identifies resources needed to adapt to illness throughout the continuum of care.* Attends Care Planning or Discharge Planning rounds to facilitate communication and the importance ofthe patient and families goals.* Participates in advance care planning initiatives.* Bases social work actions on the code of ethics by the National Association of Social Workers (NASW) by applying principles of purposeful, organized thinking to make sound judgment/decisions.* Documents measurement and evaluation of social service consultation and satisfaction outcomes aspatient progresses towards desired goals.* Evaluates and responds to the needs of clients, clinicians and community. Reviews decision making with Lead/SW II when appropriate.* Manages time and resources for complex multi-system patients.* Collaborates with team using leadership and communication skills.* Performs other duties as assigned.Concord Hospital is an Equal Employment Opportunity employer.
