As a member of the Hospice House team, the LPN works primarily performing tasks and responsibilities within the framework of case findings, health teaching, health counseling and provision of supportive and restorative care under the direction of a registered nurse carrying out the nursing process. The LPN works under the direct supervision of the RN or licensed physician and interacts with patients, spouse, significant others, family members, physicians, and fellow team members in keeping with the mission and mandate of Hospice. This specific LPN position will be working within our new Hospice House (IPU - Inpatient Unit)! As an IPU LPN, you will work closely with the staff RNs to provide quality patient care in a fast paced setting. We are looking for an LPN who is committed, passionate and looking for their next opportunity!!5% Night Differential Included!Responsibilities Include:As a member of the hospice interdisciplinary team, provides input (including any changes in condition) and supports the decisions and direction of the team as outlined in the hospice plan of care and as directed by the primary RN Case Manager.Make observations and collect data both by interview and physical nature, to identify needs and/or problems and selection of standards.Provides anticipatory guidance and support to the patient/family, according to the interdisciplinary plan of care and as directed by the primary hospice RN Case Manager, Social Worker, Pastoral Care, and/or Coordinator of Patient Care.Record monitored information (i.e., I&O, VS).Participate in patient/family instruction and documentation of instruction and patient understanding on progress notes or teaching protocol.Communicate a daily report on each patient assigned to patient's case manager.Perform phlebotomy as needed.Perform ADLs as needed.Qualities we are looking for:Requires flexibility to meet assigned tasks.Ability to effectively communicate verbally (in person and by phone) and in writing.Power to work effectively and cooperatively as a team member.Ability to be compassionate and care for those at the end of life in a dignified wayCapability to prioritize and handle multiple tasks efficiently.Highly developed organizational skills.Capability to lift/carry at least 25 poundsCan work within a fast-paced environmentEducation and Experience Desired:New York State licensed as a practical nurse.Minimum of two years experience in nursing - working in ICU, facility or a hospice setting desirable.Valid NYS driver's license with reliable transportationBasic knowledge of EMR systems with an understanding of how to use a computer, phone and various softwareHours: Rotating Schedule (7:00pm-7:30am)Full Time is 3 shifts/week with every other weekend rotation and holidaysPart Time is 2 shifts/week with one weekend a month and one holiday a yearPay:$28-29 per hourFLSA: Non-ExemptBenefits: Medical, Dental, Vision, Company Matched 403b, Tuition Assistance and more!Hudson Valley Hospice is an Equal Opportunity Employer.
