Position Summary:
The intake associate provides an orientation to the new patient and needed information/education to new patients, so they understand how to access the full range of services at Urban Health Plan.
Minimum Qualifications:
* High School Diploma required.
* One year of experience in office setting with clerical duties required. Training provided on health care administrative duties.
* Bilingual Spanish/English required.
* Must have excellent communication skills and ability to provide exceptional experience to patients.
* Understanding of phone etiquette and standard office procedures.
Key Duties:
* Conduct intake for new patients at assigned site.
* Complete patient assessment including screening questions.
* Complete required new patient documentation.
* Assure patient confidentiality.
* Participate in quality improvement program.
Starting Compensation: Minimum $18.75 per hour; Maximum $18.75 per hour

Work Environment Type, Location, and Schedule:
Applicants must have availability to commit to a schedule of 40 hours/week working between the hours of 7am-6pm including Saturdays.
