Position SummaryThe LVN Intake Coordinator function is to facilitate the referral intake process, coordinate care with the interdisciplinary group, the patient, their family, and the referral source.As a Intake Referral Coordinator, you will:
- Receives and appropriately documents new referrals in a timely manner.
- Verifies each patient's payor source and maintains and tracks insurance authorizations and initial certifications.
- Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
- Provides effective communication to patients, staff members, other health care professionals and referral sources.
- Responds to Agency and patient needs in a professional and creative manner.
- Demonstrates commitment, professional growth and competency.
- Participates in Agency-sponsored in-service training.
- Participates in the Agency's QAPI program.
- Promotes the Agency philosophy and administrative policies to ensure quality of care.
Requirements
- Minimum one year of experience in referral intake is preferred
- Experience with third party payers, Medicare and Medicaid highly desirable
- Medical Field background preferred, including familiarity with medical terminology
- Excellent computer and customer service skills
- Valid Driver's License
- Valid Auto Insurance
- HomeCare HomeBase experience preferred
