The Intake Coordinator will bridge access to care for patients by combining the roles of Hospice Intake and Supportive Care Coordination. The Coordinator will manager referrals, schedule admissions, and coordinate liaison visits, prioritizing exceptional service to referral sources. Additionally, the Coordinator will oversee Durable Medical Equipment orders and tracking, manage medical staff schedules, process referrals promptly and ensure adequate support for patient visits. Your friendly and courteous demeanor is crucial in interactions with patients, families and referral sources. Office Location: Pasadena, MD Status: Full-time, 40 hour work week. Salary: Competitive, commensurate with experience, education and credentialing Main Responsibilities:
  • Explain the differences and benefits of supportive care and hospice services to patients and families.
  • Coordinate medical staff and admission nurse schedules for both Chesapeake Supportive Care (CSC) and Hospice of the Chesapeake (HOC) patients to ensure safe, therapeutic, and efficient care delivery.
  • Foster positive communication with internal and external clients, maintaining positive relationships with staff and referral sources.
  • Demonstrate effective multitasking abilities while utilizing computerized scheduling programs and Electronic Health Records programs
  • Maintain professionalism and compassion when interacting with patients and family members.
  • Verify insurance coverage and ensure all insurance information is accurately entered into the patient's medical record.
  • Respond promptly to provider staff for emergent patient visits.
  • Provide necessary documentation for providers to complete their visits and process all referrals promptly, adhering to scheduling guidelines.
  • Ensure all patients are contacted promptly for scheduling and update the EMR with visit details.
  • Respond to referral calls, provide appropriate information, and complete associated paperwork and records.
  • Order Durable Medical Equipment (DME), track and document appropriately, and communicate with DME companies, attending physicians, and care facilities personnel as needed.
  • Conduct financial verification for Medicare patients and facilitate proper billing of hospice services.
  • Enter referral information into electronic medical record software and compile accurate admission packets based on patient diagnosis and payer source.

QualificationsYour Qualifications:
  • High school diploma or equivalent is required; an associate degree in a related field is preferred, although candidates with commensurate experience will be considered.
  • A minimum of one year of experience in hospice, home health care, or a customer service-related position is preferred.
  • Demonstrate exceptional customer service.
  • Exhibited understanding of and acceptance of the hospice philosophy.
  • Demonstrated ability to work effectively both independently and as part of a team.
  • Proficiency in multitasking in a busy environment, managing multiple assignments, and meeting deadlines with attention to detail, efficiency, and accuracy.
  • Ability to communicate effectively with medical professionals across various disciplines.
  • Basic computer skills are required, including proficiency in Microsoft Word and Outlook.
  • Demonstrated ability to efficiently multitask in a busy and stressful environment with a focus on detail and accuracy.
  • Strong customer service skills are essential.
  • Proficient in computer applications such as Microsoft
Requirements: Current Maryland driver's license and a good driving record. Reliable transportation a must.
