This position is included in a bargaining unit of Housing Works' New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union ( RWDSU ). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a CBA ) between Housing Works and the RWDSU Compensation Range: $24/hr - $26/hr commensurate with experience Overview: The Housing Works Emergency & Transitional Housing Program (ETH) provides time limited transitional housing and support services to individuals reentering the community following periods of incarceration or other justice system involvement. Program participants engage in a variety of onsite services, including case management, vocational training, housing advocacy and mental health support that address barriers to accessing housing in the community and help build skills that participants can use to achieve and maintain stable functioning in the community.Reporting to the Assistant Director, the Intake Coordinator plays an integral role managing program referrals and coordinating admission. These activities include managing the program waitlist, collaborating with internal and external referral partners, screening and reviewing referrals for eligibility and program site placement, data entry and record keeping and managing the initial admission and enrollment activities. This is an ideal position for professionals with social services and data entry experience seeking to further develop skills and experience in the reentry and housing sector. They will have the opportunity to work along side like-minded professionals committed to improving outcomes for justice-involved individuals.Responsibilities: Primary:
  • Closely review and monitor program census and submit daily vacancy reports.
  • Review and screen referrals received for the ETH Program and determine eligibility and appropriate program site placement.
  • Assist with gathering additional supporting documentation or medical records when necessary to determine eligibility.
  • Coordinate with the interdisciplinary care teams when necessary for further evaluation and screening of referrals received to determine level of care needs.
  • Manage the program waitlist utilizing the electronic records systems in place.
  • Coordinate with the referral partners or candidate for admission when there is a vacancy.
  • Facilitate recurring meetings with key stakeholders and referral agencies to provide status updates for submitted referrals.
  • Complete daily, weekly, monthly and quarterly reports timely and accurately.
  • Monitor and report referral and placement outcomes.
  • Ensure data entry is accurate and timely.
  • Create new charts in the electronic medical records system for all newly arriving clients.
  • Promote Housing Works' core values in day to day activities.
  • Performs other tasks as assigned by supervisor
Secondary:

  • Actively participate in supervision and professional development trainings.
Minimum Requirements
  • Bachelor's Degree required in social services, psychology, social work or other related field or Associate's Degree with 2+ years of social services or data entry experience.
The following are qualifications and requirements of the position:
  • Data entry experience.
  • Experience working in a heath care or non-profit setting.
  • Strong written documentation and communication skills
  • Good problem-solving skills and ability to work autonomously.
  • Proficiency in Microsoft Office Suite, particularly Excel, required.
  • Experience utilizing EMR systems.
The following additional competencies are viewed as important to success in this position:
  • Preferably bi-lingual in Spanish.
  • Excellent written and oral communication skills.
  • Excellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems.
  • Ability to work independently and collaboratively with the team.
  • Dependability and reliability to arrive and depart timely for scheduled shift.
  • Ability to frequently travel within the boroughs via public transportation.
