Job Type Full-timeDescriptionAs the Intake Coordinator, you'll be at the heart of patient care coordination, working closely with patients, families, healthcare providers, and internal teams to ensure all intake processes are completed efficiently and with the highest standards of care. You'll help patients and their families navigate the complex healthcare landscape while supporting our organization's mission and compliance standards.Key Responsibilities:
- Handle incoming patient referrals, ensuring all necessary information is collected and processed smoothly.
- Assist in face-to-face meetings with patients and families, guiding them through the admissions process and available services.
- Organize medical records, referral documents, and ensure all paperwork is accurate and up to date using Smart Sheets.
- Perform benefits checks, verify insurance coverage, and handle pre-authorizations as required.
- Communicate regularly with the Community Relations and Clinical Teams to ensure all patient information is up-to-date.
- Ensure all necessary legal documents, advance directives, physician orders, and admission forms are completed.

- High School Diploma or Equivalent (College classes or degree preferred), healthcare background and Electronic Medical Records (EMR) experience is a plus!
- Strong verbal and written communication skills, digital literacy, and proficiency in Microsoft Office software.
- Experience in managing multiple tasks and staying organized is essential.
- The ability to adjust to changing priorities and tasks in a fast-paced healthcare setting.
