*** FRIDAY - SUNDAY POSITION ***Position Title: Intake NurseJob Description:We are seeking an Intake Nurse to join our team. The Intake Nurse will be responsible for conducting comprehensive assessments of individuals seeking admission to our program, gathering pertinent medical information, and collaborating with the treatment team to develop personalized care plans.Responsibilities:
  • Conduct intake assessments: Meet with individuals seeking admission to the facility to gather information about their medical history, substance use, and mental health concerns.
  • Perform physical assessments: Conduct thorough physical examinations to assess the overall health and well-being of individuals, identifying any medical issues that may impact their treatment.
  • Coordinate with treatment team: Collaborate with clinicians, counselors, and other healthcare professionals to develop individualized treatment plans based on assessment findings and clinical recommendations.
  • Provide education and support: Offer education and support to individuals and their families about substance abuse, addiction, and the recovery process, addressing any questions or concerns they may have.
  • Document assessment findings: Maintain accurate and detailed documentation of assessment findings, treatment plans, and ongoing progress notes in accordance with facility policies and regulatory requirements.
  • Ensure confidentiality: Maintain confidentiality of patient information in accordance with HIPAA regulations and ethical guidelines.
Qualifications:
  • Nursing license in good standing.
  • Experience working in a healthcare setting, preferably in substance abuse treatment or mental health.
  • Strong assessment and critical thinking skills.
  • Ability to communicate effectively and compassionately with individuals from diverse backgrounds.
  • Knowledge of substance abuse treatment modalities and evidence-based practices.
