Job Type Full-timeDescriptionThe Clinical Intake Specialist performs Child & Adult Bio-psychosocial Assessment and Eligibility Determination for intake into all MFS services. Assists in planning, coordinating, and monitoring intake practices for both Child and Adult outpatient therapeutic, rehabilitative, support and community integration services.RequirementsResponsibilities:1. Manages the referral of consumers to MFS including screening, intake and referral.2. Provides comprehensive bio-psychosocial assessments and determines eligibility for Community Services Program services.3. Completes ANSA and consumer Certification for Eligibility for CSP Case Management and therapy .for consumers.4. Cooperatively, with the consumer and other appropriate parties, develops individualized service plan (ISP) to include treatment options for mental illness.5. Cooperatively, with the consumer, develops individualized Crisis Plan that allows the consumer to manage emerging crisis, and to identify resources for maintaining the individual in the community, to the greatest possible degree, during periods of crisis.6. Facilitates and coordinates the individual's treatment planning to assure maximum impact of the services and treatment provided.7. Referral of specialized treatment options as defined in the Individual Service Plan, and participate in the interdisciplinary teams of these providers to the degree possible.8. Assesses effectiveness / efficiency of services provided to her/his consumers and others.9. Assesses consumer satisfaction with services.10. Receives regular supervision from the CSP Manager and Director..11. Participate actively in peer and group supervision.12. Collects and provides relevant data to Department Directors as needed.Other Expectations:1. Maintain the consumer's case record information and documentation in an accurate, timely and professional manner.2. Assists the agency's ES (Emergency Services) Team in the event of a community disaster or crisis of significant community impact, or if the ES Team is faced with a circumstance in which they require additional staff support.3. Will participate and coordinate with members of the Community Network/Satellite Teams where appropriate.4. Maintain appropriate auto insurance coverage (as determined by the employee and their insurance company) for use of a vehicle for ES disaster or crisis support, and protecting themselves from action in the event of an accident during the performance of these duties.5. While every attempt will be made to foster recovery, it may be appropriate and required that consumers be transported in the employee's personal vehicle for emergencies.
