DescriptionImmediate Opening: Intake Specialist Job Summary: Office-based, Full-Time, & HourlyUnder the supervision of the Director of Skilled & Private Duty Home Care the Intake Specialist promptly receives, evaluates, responds, coordinates, communicates, and determines the appropriateness of departmental referrals and inquiries for both skilled and private duty services. Obtains and maintains accurate records for the start and resumption of care, including insurance coverage and initial authorizations. Follows up on pending referrals, and unresolved referrals and authorization issues. Assists with the planning and initiating schedules for clients receiving skilled home care.RequirementsDuties and Responsibilities:
- Receives, responds, and processes all requests/referrals for services.
- Obtains client demographic and insurance information including name, location, payer, and requested hours/types of service.
- Assists with the planning and initiation of schedules for clients receiving skilled home care.
- Creates and accurately manages new electronic charts, uploading start of care and insurance information, as well as other critical data into the EMR.
- Enters accurate service type information into the client service group.
- Verifies and inputs all insurance information prior to admission and monthly, communicating any changes timely to Finance and Departmental Director(s).
- Obtains and verifies all initial prior authorization for start of care services, communicating to field team members.
- Schedules according to authorization all assessment, start of care, and resumption of care visits for skilled and private duty services.
- Confirms primary and secondary physician support, as required by CMS, including, NPI, face to face encounters and signed orders, accurately documenting in the EMR.
- Ensures all initial documentation is within regulatory guidelines.
- Collaborates with referrals sources, serving as an educational resource for physicians and physician offices
- Cooperates with hospital and skilled nursing facility discharge planners, maintaining records of clients pending discharges.
- Uses EMR reports to ensure client service groups, initial orders, and initial schedule are correctly entered and maintained.
- Manages inquiry reports that include agency non-admits for skilled and private duty.
- Provides a detailed weekend/holiday client status report to clinicians on-call.
- Demonstrates teamwork, supporting other department functions, including authorizations, skilled scheduling and order processing, as needed.
- Demonstrates awareness of, follows all safety policies and procedures, and adheres to recommended safe work practices. Also contributes to the control of hazards in specified area of responsibility.
- Performs other duties or special projects as required or as assigned, including on-call if needed.
Education/Training:
- High School Diploma/GED
- Pass background check
- Maintain a valid Ohio driver's license, active auto insurance, and good driving record.
