Overview of Position: The Intake Specialist is an essential member of the medical team and acts as the coordinator for the internal referral process.
Qualifications: The Intake Specialist must be able to work in a multi-disciplinary team and model healthy work practices. Must express a commitment to support and implement HopeHealth's mission, vision, values, policies and procedures.
Knowledge/Skills: Experience in a medical office setting. Excellent written and oral communication and computer skills. Strong interpersonal skills and ability to multi-task.
Essential Job Functions:
* Navigate incoming referrals to departmental providers.
* Schedule new patients with the appropriate medical provider and ensure the receipt of medical records as required to complete office visit.
* Schedule outgoing referrals to include MRI and other required medical screenings and / or procedures.
* Keep documentation current through timely scanning and filing.
* Patient Check in / Check out and demographics verifications; updates are to be completed as appropriate.
* Answer patient phone calls and assist with follow up calls to ensure compliancy with patient care plans.
* Maintain on-going communication with staff through documentation as assigned
* Complete all records and reports as assigned.
* Participate in staff meetings and in-service training as required.
* Other duties as assigned.
Physical Requirements: Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
