POSITION: Intaker/Case Manager Homelessness Prevention (FT, Non-Exempt)REPORTS TO: HPS SupervisorCOMPENSATION: $24.00/hr. to $27.00/hr. depending on skills and experiencesJOB SUMMARY:ICAN, located in San Jose, CA, is looking for a bilingual and bicultural English/Spanish or English/Vietnamese Case Manager to support our HPS (Homelessness Prevention System) Program.The ideal candidate will be supporting families at-risk of homelessness through: financial assistance; temporary case management; and connections to resources and supports in the community. This individual must be: 1) a self-starter; 2) highly organized and detail oriented with strong communication skills; 3) experienced in case management; 4) understanding of and sensitive to the needs of low-income families; and 5) able to work flexibly, creatively, and with a growth mindset.The HPS Case Manager will be working with low-income families to prevent or end homelessness by: 1) screening, assessing, and conducting intakes from prospective participants; 2) enroll families into the HPS program with all required documentations collected and completed; 3) provide enrolled families with temporary financial assistance; 4) provide families with strength-based case management to help them gain or maintain permanent housing; 5) connecting families to other community resources.ORGANIZATIONAL BACKGROUND:ICAN was founded in 2000 by three refugees of the Vietnam War with the mission to lend a helping hand to the families, friends, neighbors and fellow countrymen whom they left behind. Outreach and Engagement: (5%)Conducts client outreach at various community eventsEstablishes and maintains relationships with other community support agencies to network client servicesAttends all required agency, staff, and community-based meetings and trainings as neededPrescreening, assessment, and Intake: (55%)Conducts prescreens and intakes to determine participant eligibility and assess appropriate level of assistance needed.Conducts HPAT interview and assessment to determine eligibility of HPS programCollects and compiles necessary documents to verify eligibility and needs of clientCase Management: (30%)Manages a flexible case load of up to 30 or more households, including households receiving one-time financial assistance and households participating in case managementProvides case management to individuals and families by establishing goals and strategies to achieve clients housing stabilityProvides referrals in order to secure necessary services and support for clients: medical, Li- HEAP, job training, housing, Law Foundation, etc.Assists families with finding and securing new housing, when neededAssists families with immediate crisis prevention, advocacy, problem-solving and other appropriate interventions, as neededProvides client follow up to maintain and monitor housing stabilityStay informed of community resources available for HPS clientsData Management and Quarterly Reporting (10%)Maintains case files and progress notes, adhering to professional standards and designated time framesMaintains accurate, updated, confidential case files on ICANs secured databaseDocuments program activity, including prescreens, intakes, financial assistance, and case management into HMIS within one to 3 business day of activitySubmits quarterly narrative, data and other reports as neededPerform other duties as assignedREQUIRED EDUCATION & SKILLS:
- Four-year degree in social work preferred or related degree with case management experience.
- Bilingual and bicultural in English/Spanish or English/Vietnamese is required. English/Spanish or English/Vietnamese speaker, with ability to read and write in English/Spanish or English/Vietnamese.
- Strong organizational skills; ability to plan, organize, prioritize and multi-task. Ability to work with little direct supervision.
- Strong communication skills, including the ability to deliver difficult messages with compassion and to communicate respectfully with families in stressful situations.
- Ability to thrive in a flexible, fast-paced environment, while maintaining a positive, solution- oriented approach
- Computer skills including proficiency in MS Word, Excel, PowerPoint, Google Suite, Adobe, and the ability to quickly learn and develop proficiency with tracking database systems

- Candidate should be passionate and have commitment to building communities and promoting civic engagement.
- Candidate should have a willingness to learn and a growth mindset.
- Experience working with persons from diverse ethnic and cultural backgrounds who are economically disadvantaged.
OTHER REQUIREMENTS:
- Must be able to lift 25 pounds as the service is sometimes physical. For example, setting up for events which may require transporting or lifting equipment and materials, e.g., easels, flip charts, refreshments, brochures, chairs, tables, etc.
- Must be able to report to service site on a regular and reliable basis, and able to travel between sites and to attend offsite events throughout the county, as appropriate
- Must be able to work a flexible schedule, which includes evenings, weekends, and holiday hours, as appropriate, to coordinate and provide services and activities for families during non-traditional hours.
LICENSES, CERTIFICATIONS, REGISTRATIONS:Must possess a valid California Drivers license, reliable transportation and current auto insuranceOTHER QUALIFICATIONS:
- Must be at least 18 years of age with permit to work in the US
- Clearance of fingerprint background check
- Must have valid TB screening and COVID19 Vaccination
