Job Description:
This position is eligible for a $5,000 sign on Bonus. You will received $2,500 after hire and received the rest after 6 months of employment.The Behavioral Health Consultant (BHC) as part of the primary care team treatment team, identifies, triages, and manages patients with medical and behavioral health problems within the primary care setting. In addition, the BHC will provide psychoeducation, patient education strategies, and targeted behavioral interventions.Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.We're looking for someone who can:
  • Provides clinical services as a BHC within the Primary Care Behavioral Health (PCBH) model of care at one and/or multiple sites.
  • Regularly consults with medical providers, medical assistants, and nurses to assist patient care in line with the patient centered medical home. Care may address mental health and substance misuse conditions, health behaviors (including their contribution to chronic medical conditions), life stressors and crises, and stress-related physical symptoms.
  • Identifies patients from patient population who would benefit from behavioral
  • Delivers individual, couples, and group targeted behavioral health interventions for all ages; provide screening, assessment, and referrals to appropriate specialty clinical services.
  • Provides consultation to and coordinate care of patients with primary care
  • Identifies, refers, and advocates for patients needing specialty behavioral health
  • Be immediately available to health center staff during assigned clinical hours in event of psychiatric emergency and/or crisis.
  • Committed to improve quality measures for target populations while also being willing to create clinical pathways at discretion.

Supervisory ResponsibilitiesWorks independently within scope of the job description with general direction and consultation from supervisor. Does not directly supervise staff. May provide work direction to other staff in consultation with leadership. May supervise and/or direct contractors, volunteers, and/or students.Work EnvironmentPrimary environment is home office, administrative office, or clinical office. This role requires regular walking. This role may come into contact with patients who may have contagious illnesses.Physical Demands
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift 15 pounds at times.

  • Master's Degree or higher in Psychology, Social Work, or related field.
  • LPC, LPCC, LGSW, LICSW, MFT, LMFT
  • Experience providing care, preferably in a community health center setting.
  • Licensed in the State of Minnesota as a mental health provider.
  • Bilingual, English/Spanish preferred but not required
