JOB SUMMARY
- Conducts comprehensive patient assessments for recovery impact
- Develops personalized, patient-centered care plans
- Engages patients and families in care through advocacy and education
- Utilizes professional judgment and motivational interviewing for goal achievement
- Provides counseling on treatment decisions and end-of-life issues
- Coordinates safe patient transitions between care settings
- Advocates for appropriate services within health plan benefits
- Collaborates with the treatment team for complex cases and care conferences
- Evaluates patient responses and modifies care plans as needed
- Participates in process improvement activities for risk reduction
- Facilitates interventions in cases of abuse and assault
- Supports department goals for organizational success
- Performs assigned duties and responsibilities.
JOB REQUIREMENTS
- Clinical background
- RN or LMSW with valid, unrestricted State of Michigan license
- 3-5 years of case management experience
- Strong PC skills
- Experience in discharge planning, home health care, rehabilitative medicine, community health and/or managed care
- Knowledge of preventive service guidelines, clinical practice guidelines, behavior change theory, Medicare and Medicaid regulations and case management principles.
- Knowledge of medical ethics and legal implications related to case management.
- Understanding of social determinants of health and their impact on a patient's wellbeing.
