- Foster a safe and supportive environment that encourages and enhances independence, community integration, and individual personal growth.
- Enhance community engagement and individual socialization.
- Identification of transportation needs and preferences.
- Safety education and training on pedestrian practices.
- Personal safety and emergency preparedness training.
- Role-playing scenarios for handling unfamiliar situations.
- Assistance in identifying and accessing healthcare, education, and employment services.
- Research on available resources and guidance on eligibility criteria.
- Support in building relationships with supportive peers, family, and community members.
- Facilitation of introductions and socialization skills training.
- Training on effective communication strategies, active listening, assertiveness, and conflict resolution.
- Role-playing exercises and real-life applications.
- Identification of individual needs and interests for leisure and social activities.
- Facilitation of participation in community groups or clubs.
- Identification of healthcare needs and support for accessing appropriate services.
- Coordination with healthcare providers and scheduling appointments.
- Cueing, guidance for routine self-care activities.
- Training and hands-on assistance as needed.
- Identification of strengths and building resilience.
- Development of individualized strategies and care plans.
- Regular monitoring of progress toward goals.
- Assistance in developing knowledge and skills for maintaining a safe household.

- Daily assistance with nighttime safety measures.

- Cueing, guidance for routine household tasks such as cleaning and organizing.
- Support in completing routine household tasks such as cleaning and organizing.

- Assistance in maintaining tenancy and navigating household complexities.
- Collaboration with property management to address issues.

Qualifications:
- High school diploma or equivalent (some positions may require additional education or certifications)
- Previous experience in a similar role or working with individuals with developmental disabilities, mental health concerns, or related populations is preferred.
- Demonstrated ability to provide compassionate, person-centered care and support.
- Strong communication and interpersonal skills with the ability to work effectively within a team.
- Ability to maintain professional boundaries, confidentiality, and ethical standards.
- Basic computer skills and proficiency in documenting services accurately.
- Physical ability to assist individuals with mobility, lifting, and personal care needs as required.
- Must have a valid driver's license with an acceptable driving record.
- Current auto liability insurance.
- Ability to drive own vehicle while working.
- Acceptable clearance of background check.
