Job Type Full-timeDescriptionJob title Case Manager /Intensive Case Manager Reports to Case Manager Supervisor Position SummaryThe case manager will determine resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the RHD values should be demonstrated as job duties are performed.Essential Duties and FunctionsTeam-Based PlanningAct as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant.Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.Direct Client CareProvide community-based services to program participants focusing on education, connection with resources, community interaction and achievement of recovery plan goals.Administrative FunctionsComplete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.Other duties as assignedEssential Duties AddendumOn-Call after hours/weekend/holidays on a rotation basis as required by regulations Linkage and Referral to Community ResourcesAssessment and Goal PlanningGaining Access to ServicesMonitoring of Service DeliveryCrisis Planning and problem resolutionInformal Support Network BuildingFollowing all Operational Procedures and funder/licensing body regulationsAssisting clients in attending community appointments as needed/determined by goal planMoving Clients into their residences (Crossroads PSH clients)Meeting weekly productivity goals as set by the programTimely completion and submission of all case notes and required paperwork Other duties as assigned QualificationsBachelor's Degree in Human Services or related field.Intermediate computer skillsOne year Case Management experience preferredTwo years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services preferredValid driver's licenseWorking conditionsAdditional Special Working Conditions:Working out in community within the service delivery area of the program. Required to drive program participants to appointments.
