The plan covers all medically necessary and preventive services covered under Medicare. They are strongly focused on improving their members' physical, mental, and social well-being through personalized clinical care and therapies. They are currently hiring due to growth.POSITION DESCRIPTION: The provider is responsible for managing a panel of SNF residents, focusing on treating patients in place in collaboration with a full primary care team.The provider conducts comprehensive assessments, develops and delivers individualized plans of care, attends care team meetings, communicates with family members / caregivers, and ensures the care team understands the patients goals and needs of care. The provider is supported by a Clinical Manager / Clinical Director, as well as a local Medical Director.Specific responsibilities include, but are not limited to:- assess and plan healthcare that is individualized to the patient- review and interpret medical, surgical, and social histories- perform screening procedures for patients- interpret laboratory and other diagnostic data and recommend treatments or actions- reconcile medications- be responsible for prescribing or performing various treatment modalities- work collaboratively with physicians, care coordinators, facility staff, family members, etc.- perform direct care management support- educate care teams on patient needs and early identification of changes in condition- participate in organizational meetingsThe practice uses a mobile-based EHR.The clinical work is autonomous, with one-on-one patient visits and a high level of independence.
