Interventional Care Nurse (I-Care RN)
Reports to the Director of Nursing/Administrator
Summary: The I-Care nurse is responsible to audit all tasks, documentation compliance, orders, care plans, direct care techniques related to nursing restorative rehab, intravenous therapy and respiratory therapy and participate in the education and competency of Certified nurse aides or therapy aides and licensed nurses performing the therapy and/or treatment modalities.
Qualifications:
· Must have NYS Licensure in RN with state license required.
· Preferred at least three years of clinical experience with intravenous treatment management, restorative nursing programs and respiratory care and treatment management
Essential Functions:
Responsible for, but not limited to:
Decreasing negative resident incidents by improving function and minimizing deterioration. The role will complete this goal by focusing on the provision of targeted respiratory, restorative nursing rehabilitation and intravenous therapies This role will provide resident oversight and monitoring in these key areas.
· Upon admission, readmission and reevaluation as indicated, each resident's respiratory status and risk factors relevant to nursing rehab and intravenous treatment will be assessed by an RN and medical professional.
· Initiate all appropriate respiratory. nursing restorative and intravenous therapy orders, tasks, consults and care plans as needed.
· Review all current orders, task, care plans and documentation to ensure they are appropriate
· Document periodically the rationale for current treatment and continuation of treatment in the resident's medical record
· Document pre and post assessments of all respiratory treatments daily and ensure assessment is continued on days the employee is not scheduled to be at the facility.
· The ICN will review all administration records monthly and at time of discontinuation of treatment that to ensure there are no missing signatures or errors.
· The ICN role attend all relevant interdisciplinary meetings
· To discuss residents with acute changes, active treatments or noted with declines that may benefit from a nursing restorative/ maintenance program
Required Skills/Abilities:
Organized manner to maintain schedule and generate reports in a timely fashion.
Reassuring manner to deal with patients of all ages.
· Prolonged periods of sitting at a desk and working on a computer.
· Prolonged periods of standing, bending and reaching.
