Job SummaryThe Jail Based Addiction Counselor position is part of the SHIP (Sheriff's Homeless Improvement Project). This position is in a seperate wing of the Albany County Jail. The program provides the homeless a roof, food, access to other basic services, leads on steady work and the support they need to become successful members of society. You will play a crucial role in assisting the individuals struggling with substance abuse disorders. Your primary focus will be on providing counseling and support to help clients overcome addiction and develop healthy coping mechanisms.Responsibilities1. Provide counseling services to clients on understanding of addiction, and application of addiction counselor knowledge to practical treatment of clients.2. Provide addiction education and relapse prevention information to the Men and Women residing in the SHIP Program, Recovery Group through Interface meetings.3. Coordinate with medical, mental health, addictions, correctional staff, and any pertinent agencies regarding within and outside the facility.4. Maintain excellent record keeping, tracking all activity and reporting back to pertinent partners on progress and other shared information.5. Complete Department of Health and other agency surveys, documents and forms.6. Attend bi-weekly meetings including case conference with treatment providers to streamline coordination of services to assist in the discharge planning and progress of referrals.7. Prepare bio-psych-social, LOCADTR reports for inpatient residential recommendations and obtaining approval from Central Management Unit of the Department of Social Services.8. Coordinate phone screenings and work with other staff assigned to clients in other case management capacities.9. Schedule appointments for clients to be seen by Catholic Charities and other providers.Requirements1. Working knowledge of addiction with CASAC or CASAC-T credentials a plus. Peer Advocate with at a minimum of one (1) year experience a must.2. High school diploma or GED, some college; completed CASAC Credentialing training.3. Valid Driver's License.
