ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: This is not intended to be an exhaustive listing of job functions. This job description is in no way states or implies that these are the only duties to be performed by this employee. The employee is required to follow any other instructions and to perform any other duties as assigned.1. Regular and predictable attendance is an essential requirement of this position.2. Manage timely completion of vocational reports. Ensure that vocational services are documented completely, concisely and at intervals required by funding sources. This includes regular progress notes, activity logs and incident reports per agency guidelines.3. May be responsible for completing job retention follow up activities and job retention documentation for individual placements.4. Attend periodic reviews and other meetings related to consumer treatment.5. Work with consumer to develop and implement strategies of work simplification and environmental modifications.6. Provide orientation, training and follow-up services at work sites. Provide retraining as required and needed to individuals and or groups.7. Act as a role model for program participants and demonstrate solid employability skills.8. Provide direct assistance to consumers seeking employment (job clubs, job leads, application submissions, interviewing skills, etc) in conjunction with the Employment Training Specialist (ETS).9. Develop goals that address on the job safety as evidenced in the IPOS. Review work site emergency procedures and provide necessary training and support.10. Other duties as assigned for the good of the consumers, the program, the department, and the agency.11. Provide input as requested in the development of the consumer's treatment plans and goals.12. Implement and participate in the clinical and behavioral goals for each consumer using approved methods of instruction and intervention.13. Coordinate multiple priorities and have the ability to change tasks as needed to meet the needs of the program and or participants.14. Ability to plan, schedule, and implement vocational programming individually or for large and small groups in the facility and or community setting including production work, skills trainings, community job set up and adaptations as needed.15. Support individuals utilizing MANDT Teachings approach and intervene in any crisis situation that arises according to established procedures. Promote physical and emotional safety and wellbeing of all individuals involved.16. In facility based program and or community based settings assist with emergency procedures per needs of person and as Plan of Service identifies and or specific community work site procedures.17. Safely transport consumers, report any vehicle maintenance needs (Hope vehicles), and have an accident (preventable) free driving record according to Agency standards.18. Communicate and record health concerns in accordance with Agency standards and program guidelines.19. Maintain a safe working environment by: using proper lifting techniques, positioning people safely, safely securing people in their wheelchairs and addressing safety issues before an accident occurs. Also responsible for reporting any accidents, filling out incident and basic injury reports as needed in a timely manner.20. Promote a positive morale among staff by modeling and encouraging effective teamwork and communications. Continually work to promote successful working relationships among all co-workers Hope Network and host site if applicable. Alert supervisory staff of situations that may require further intervention.21. Monitor, supervise, accompany and interact with consumers in their home, community, or program that is consistent with their Individual Plan of Service (IPOS) and MANDT Teachings.22. Develop and maintain appropriate/healthy professional relationships with consumers while providing consistent follow through and enforcing applicable rules and routines of the home and/or program. Develop and maintain appropriate professional relationships with family/friends/guardians.Educational / Talent Requirements:1. Minimum Education required: High School Diploma and 3-5 years of related experience working with persons with disabilities preferred in a vocational setting.2. Excellent teamwork skills and the ability to energize and coordinate diverse populations.3. Strong organizational and time management skills.4. Strong oral and written communication skills.5. Demonstrated ability to work with people with developmental and other disabilities.6. Ability to be self-directed.7. Strong computer skills including Word, Excel, and OutlookWork Experience Requirements:1. 3-5 years of experience working with people with disabilities preferably in a vocational setting.
