What We Are Looking ForJob Title: Kaplan Family Hospice Residence LPN - (Days)Schedule: Full-Time LPN to work 7:00AM - 7:30PM, 3 shifts per weekThis position is an exciting opportunity to provide care to our patients located in their homes in the Orange and Sullivan County NY area. This position works as a member of the hospice interdisciplinary team to provide support and to implement the Plan of Care initiated by the RN.Essential Job Functions/Responsibilities Provides direct patient care as defined in State Nurse Practice Act.Implements current nursing practice following an approved Plan of Care initiated by the Registered Nurse.Provide accurate and timely documentation of patient services to reflect the Plan of Care.Provide patient/family/caregiver education and information pertinent to diagnosis and individualized Plan of Care.Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members, e.g., hospice aide, OT, PT, MSW, ST, and Dietitian.Uses equipment and supplies effectively and efficiently.Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.Performs other duties as assigned by the RN.Adheres to Infection Control and other pertinent Hospice policies regarding patient family care.Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.Communicate with primary RN regarding patient's needs and reports changes in patient's condition.Teaches the patient/family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as directed by RN. Recognizes and utilizes opportunities for health counseling with patients/families/caregivers. Works in connection with the other members of hospice interdisciplinary team.Provides and maintains a safe environment for the patient.Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.Fulfills the obligation of shift assignment.Participates in Performance Improvement Program.Requirements Candidates must have a current NYS LPN License. Graduate of an Accredited School of Nursing. Applicant must demonstrate basic knowledge of nursing theory and practice, knowledge of appropriate technology and practice standards for area of responsibility and can demonstrate sound clinical skills, standards of practice and positive communication skills. Previous hospice/home care experience preferred. Reliable transportation required.Job LimitationsProlonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching.
