Location/Department: Hospital - Laboratory80/pp Evenings - M-F 11p - 7:30pJob Summary:Serves the needs of inpatients, outpatients, team members & physicians through the accurate, timely & courteous collection of laboratory specimens & other related laboratory duties.Supervisory Responsibilities: NoneDuties/Responsibilities:
- Uses only approved abbreviations.
- Understands patient safety goals & actions taken.
- Uses 2 patient identifiers before giving treatments, demonstrates process.
- Understands read back of verbal orders/critical lab values & can state process.
- Understands procedure to clarify physician orders.
- Uses standing orders when indicated.
- Accurately calls report to MD as warranted by condition; reports to other providers as needed.
- Reports sentinel events or adverse drug reactions or incidents as warranted.
- Uses final verification before invasive procedures & can verbalize correct process & documents in EHR.
- Reassesses patient in response to changes in condition after intervention as needed & documents in EHR.
- Understands patient safety goals & actions taken; demonstrates awareness of patient rights.
- Always introduce self to patient & explains procedure thoroughly to assure patient comfort & compliance.
- Competently performs all routine phlebotomy procedures.
- Observes patient & puncture site carefully during procedure for adverse reactions. Calls for additional support if needed.
- Knows proper collection tubes & procedures for routine testing.
- Ability to collect, research, & process esoteric lab testing (ex: 24 hour specimens, chromosomes, South Bend Medical Foundation, stones, etc.)
- Sends out reference lab specimen following proper procedure.
- Sends out bone marrow, flow, cytology, chromosome analysis, & pathology specimens following proper procedure.
- NP collections with use of N95 or PAPR for lab specimens.
- Reports accurately a& completely to ensure continuity of care to oncoming shift.
- Routinely performs venipunctures, rarely requiring a re-stick.
- Accurately orders all tests, rarely missing a test ordered or ordering incorrectly.
- Accurately documents test order comments & reporting method in patient's record in lab entry of the LIS system.
- Accurately documents & carries through to completion of all verbal orders received.
- Accurately handles & files all faxed orders received for future draws.
- Accurately selects the correct account for test ordering in LIS.
- Accurately & timely faxes & documents all out of system doctor reports in LIS.
- Accurately register CLIENT & pre-registered patients & cancel registrations.
- Accurately input & edit medical necessity diagnosis codes for lab testing.
- Accurately check patient charges for lab tests ordered.
- Triages workload appropriately completes procedures within the scheduled shift without sacrificing the quality of work performance.
- Performs duties independently, requiring little direct supervision, but seeks guidance as needed.
- Handles conflict situations appropriately.
- Performs daily, weekly, & monthly periodic maintenance duties.
- Maintains supplies, stocking, & keeps the draw areas clean & orderly.
- Fills physicians' office orders for supplies timely.
Required Skills/Abilities:
- Ability to communicate & work well with physician & staff.
- Must have strong verbal & written communication skills.
- Able to perform phlebotomy.
- Able to operate office equipment, i.e. computers, phones, fax & copy machine.
Education and Experience:
- High school diploma or equivalent
- Prior phlebotomy experience preferred
Physical Requirements:
- Constant manual dexterity & use of hands; frequent walking, standing, talking & occasional sitting, pushing, pulling, climbing, stooping, kneeling & reaching.
- Frequently may need to lift up to 25 lbs; 50 lbs on occasion.
