Location: The Southern Territory Headquarters - Nashville, TNAbout this opportunity:Serves as lead worker to case management staff, serves as the first point of contact for questions about services; interviews accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an emergency shelter and established transitional housing programs; understands the uniqueness of the client's history to determine most effective program plans; develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff; serves as an advocate for clients to acquire services that will enable them to cope with their environment functionally; ensures constant compliance with funding requirements.Key responsibilities:Serves as the lead worker to other case management staff. Provide training to staff.Ensures that team members are meeting impact measures in relationship to grants and contracts. Prepares and maintains case records and logs on all assigned clients; ensures accuracy and completeness; and enters pertinent information into the Homeless Management Information System (HMIS). Prepares and maintains statistical records on all services provided; compiles and prepares monthly statistical reports; ensures the accuracy and completeness of the same. Assists clients in locating/securing affordable housing; works closely with clients to transition into the new housing; refers clients to appropriate Corps programs or other agencies based on client's needs and in accordance with their program plan.Establishes unique, comprehensive short and long-term program goals (six months or more) with eligible clients based on personality, decision-making abilities, mental capabilities, addiction history, work history, housing history, etc.; explains goals to clients in a manner that is easily understood; consistently counsels client on ways to work toward financial self-sufficiency and housing stability while in the program. Conducts orientation of new staff members Completes and articulates discharge plans to clients exiting the program. Interview applicants to determine eligibility for program enrollment based on established program guidelines and requirements; conduct needs assessment, obtain pertinent information, and refer applicants to other agencies if not appropriate for the program. What we are looking for in you:Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field,AndFour years of progressively responsible experience providing direct case management social services, including accessing clients' needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources, with one-year supervisory experience preferred,OrAny equivalent combination of training and experience which provides the required knowledge, skills, and abilities.Physical Requirements and Working Conditions:Ability to meet attendance requirements.Ability to read, write, and communicate the English language effectively. Ability to communicate clearly and effectively both orally and in writing. Ability to think clearly and quickly in order to maintain control of client caseload.
