Job Type Full-time Description The Lead FC Nurse will assist in the management of the nursing staff who provide home visits to families with newborns. The goal of Family Connects home visits is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The Lead FC Nurse will direct the team of clinical staff in order to schedule, visit, and support families in the community using the evidence-based Family Connects home visiting protocol. Ability to communicate effectively with others is essential. Experience in postpartum/child health, home visiting, and/or lactation education preferred. Requirements Education:

  • B.S. degree in nursing with current RN license, or B.A. degree in another field with current RN license, or A.S. degree in nursing with state-level RN. MSN, MPH, or other advanced degree preferred but not required.
  • Nursing knowledge and 3-4 years of clinical experience relating to the field of non-profit, maternal child health and education is strongly preferred. Current or compact licensure in the state of New Jersey, current CPR certification. Ability to direct a team of nurses with beginning to intermediate level management expertise. Ability to work with diverse populations. Understand the commitment to a universal home visiting program as a public-health intervention for newborns and their mothers.
  • Preferred skills include (a) Spanish language proficiency, (b) clinical experience related to domestic violence, substance abuse and/or depression, (c) one or more years of experience in social work, or other related field, and (e) Certified Lactation Educator or Consultant, (d) ability to provide a high level of customer service with attention to customer satisfaction.
  • Computer proficiency required with Microsoft programs. Strong interpersonal & oral/written communication skills required.
  • Valid Driver's license with the ability to provide proof of ongoing automobile insurance coverage is required. This position will entail reimbursable local and statewide travel utilizing personal vehicle.
Essential Duties and Responsibilities include the following: •Become fully trained in the Family Connects home visiting protocol as stated in the current program implementation manual. •Assist in training new FC staff to the Family Connects model •Assist the Nursing Supervisor with the certification and fidelity of the nursing staff to the protocol. •Track visiting rates and nurses' fidelity to the home visiting protocol through the use of computer-generated reports and dyadic supervisory home visits. •Communicate regularly with the Nursing Supervisor with any nursing and/or program issues. •Coordinate assignments to the nurses to provide home visiting/virtual service to the target region. •Assist in providing education to community partners within the Mercer county region. These efforts may include visiting prenatal care clinics, doctors' offices, and birthing hospitals. •Assist the Nursing Supervisor with program duties as they arise. •Maintain a reduced caregiver caseload. Home Visiting Functions: The Lead FC Nurse should be proficient in performing and providing supervisory guidance for the following home visiting/virtual nurse activities: •Follow the Family Connects home-visiting protocols. •Maintain records of client observations and activities as required including documenting information for family assessments and program evaluation ideally within 24 hours of the encounter. •Provide a health and physical assessment of mother and baby in the home to determine well- being. •Assess risk factors for family, including depression, substance abuse, and domestic violence. Exercise clinical skills in appraising situations and making decisions. •Establish a trusting relationship with families during in-home visits and engage the family to establish a family support plan with goals, objectives, and activities that address family strengths and needs. •Assist parents in making appointments for doctors, social service agencies, etc. Act as a liaison between families and other community agencies, serving as family advocate as required. •Work with team to ensure appropriate referrals and connections to community resources as needed. Identify gaps in community services that may be addressed by local partners. •Apply principles of reflective supervision for clinical staff support. •Apply working knowledge of parent-child interaction, parent-child health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assist in creating a health plan for entire family. •Respond to client needs through flexible scheduling of home/virtual visits.
