We are also conveniently located just two hours north of San Francisco and 45 minutes from the Pacific Coast.Job Summary:Compounds, prepares, mixes, manufactures and dispenses medicines using a high level of technical and organizational skills as well as critical thinking ability. Oversees the proper handling, usage and storage of drugs in the pharmacy. Maintains standards set by the department and translates policies and procedures into daily operation and clinical activities. Supervises and trains assigned technical personnel and assumes responsibility for technical support activities within the pharmacy department.Job Requirements:Education and Work Experience:
- Graduate of school of pharmacy: Required
- Bachelor's Degree: Preferred
- Two years' technical experience: Preferred
- Two year's experience as a clinical pharmacist specializing in a regulatory area: Preferred
- One year's hospital pharmacy experience: Preferred
Licenses/Certifications:
- Pharmacist license in the state of practice: Required
- All licensures and certifications needed to fulfill the duties required of a Pharmacist at the state level: Required
- Registered Pharmacist (RPH): Required
Department Specific License/Certifications:
- Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required
Essential Functions:
- Evaluates, processes, distributes and dispenses physicians orders, including unit dose and IV admixture services. Effectively utilizes all available pharmacological resources in order to provide timely, accurate, patient-specific information upon which decisions may be based. Ensures pharmacy services are provided within established deadlines and department procedures.
- Supervises clinical staff and serves as a liaison between the pharmacist staff and administrative staff regarding performance and compliance with clinical protocols. Maintains compliance in the following areas: anticoagulation, chemotherapy processing and daily clinical activity bundle.
- Ensures compliance with all local, state and federal regulations and the hospital and department policy and procedures pertaining to the practice of pharmacy. Assists nursing staff in medication-related issues. Provides recommendations to physician for cost-effective use of medication. Provides in-service or educational seminars as needed to other health professionals for safe and effective use of medications.
- Assists with quality assurance monitoring, data collection and/or continual accreditation preparedness. Collects and compiles data on all medication error reports and Adverse Drug Reaction Report. Performs root-cause analysis of Medication Error findings. Makes suggestions for modifications on existing clinical polices and procedures as necessary based on finding on root-cause analysis.
- Accurately and completely documents all clinical activities and interventions. Participates in quality improvement and medication use review activities of the department. Detects and reports suspected adverse drug reactions and medication errors in a timely manner. Individuals experiencing exposed rashes, sunburn, weeping sores, conjunctivitis, active respiratory infections or other communicable disease, or those wearing cosmetics, nail polish, or artificial nails shall be excluded from the ISO Class 5 and ISO Class 7 compounding areas until their conditions are remedied.
