We are looking for an RN for our Licensed Home Care team.Department & Position Overview:We are looking for compassionate, empathetic Nurses to assess our patients current needs.Every Day You Will:Provides care and services according to scope and standards of Nursing Practice and adheres to the Code of Ethics for Nurses. Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency's compliance program and privacy policy. Follows the agency's policy and procedure regarding infection control measures through proper handwashing, bag technique, use of Personal Protective Equipment, principles of aseptic technique, exposure control/reporting, care of equipment, safe needle technique and other protocols consistent with agency's infection control program. Assesses and regularly reassesses the client's needs and his/her response to care, treatment and education.Develops implements, evaluates, revises, and provides input to the client's plan of care to include the physician's plan of treatment, problem list, nursing care plan and the aide's plan of care. Provides care according to the plan of care. Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner. Actively coordinates the care of the client through communication and written documentation to the physician, case manager, and supervisor, other disciplines involved in the client's care, labs, equipment, or supplies, and community resources.Supervises ancillary staff involved in the client's plan of care.Provides education to clients and families through assessment of the learning needs, development of teaching objectives and plan, implementation of the teaching plan, evaluation and revision of the teaching plan as needed. Interprets the philosophy and objectives of the agency to clients, families, and others. Participates in the agency's Performance Improvement Program. Performs other job-related duties as may be required or directed by the supervisor. Ideal Candidate Will Possess:At least an Associates Degree and current NYS License that is in good standing3+ years Nursing experienceAbility to travel within the 2 boroughsCurrent knowledge of state and federal home health regulations and community service systems proficient in Microsoft (Word, Excel, Outlook)Excellent verbal and written communication, organizational and time management skillsNice-to-Haves:Bilingual in English and one of the following languages: Spanish, Russian, Chinese, Korean and Bengali Experience in Home Care, Geriatrics and Case Management; field experience assessing patient careWhy work at FreedomCare?We are here for you and committed to providing a best-in-class employee experience. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $100.00 and $130.00 per assessment at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.Salary Range (Per Assessment)$100-$130 USD
