Licensed Clinical Social Worker, Federally Qualified Health Center Do you enjoy working with families? Are you interested in Social Work, or looking to make a career change? Make a difference in the lives of at-risk children and families with the School Based Clinical Team. Longstanding FQHC isseeking a Licensed Clinical Social Worker to join their School Based Health Center team. The School Based Clinician will complete psychosocial intake & ongoing assessments. Complete person-centered treatment plans prioritizing goals & objectives. Provide office-based individual/family and/or group therapy with children and/or adults presenting with mental health and/or substance challenges. Participate in Interdisciplinary Team Meetings as well as other pertinent meetings/appointments to ensure all team members are continuing open communication of the individuals progress. Collaborate with other providers and/or community resources to support community mental health needs. Support and complete step-down/aftercare/discharge plans. Full or Part-time.
