Job Summary: This Licensed Clinical Social Worker position is responsible for working directly with families as part of the diagnostic process for developmental evaluations and on-going care for children with neurodevelopmental differences. Responsibilities include completing diagnostic and behavioral intake assessments, providing follow-up appointments upon an autism spectrum diagnosis to navigate families with the next steps, interacting and connecting families with other service providers, county resources and state agencies (e.g., Office for Persons with Developmental Disabilities), and working with families needing assistance to advocate for school services. Additional roles include crisis management, case management, family support, and coordination of services via a triage process to ensure patients have access to the necessary service providers. Duties include involvement in the on-going care coordination and therapeutic service delivery (e.g., parent education) for existing patients served at the Golisano Center for Special Needs (GCSN). Participates in working with an interdisciplinary team (e.g., psychologists, pediatricians, nurses, etc.) to review referrals and facilitate appropriate service delivery within the Department of Pediatrics. Minimum Qualifications: MSW from a Social Work program accredited by the Council on Social Work (CSWE), valid NYS professional Licensed Clinical Social Worker (LCSW). Three years of post-licensure relevant experience working with interdisciplinary teams in the field of pediatrics. Effective report writing and computer skills are required. Preferred Qualifications: Relevant experience working with children with neurodevelopmental differences. Demonstrate the ability to work on interdisciplinary teams.
