Position Overview: The Mental Health Therapist will be responsible for assessment of clients including the development, evaluation, and modification of the plan of care. The clinician interprets and performs complex patient care procedures specific to their area of expertise. Sets objectives and goals for treatment and develops treatment plans in conjunction with the medical director, Implements and monitors treatment plans. Provides individual or group treatment, such as psychotherapy, supportive therapy or crisis intervention on a long term or short-term basis to various populations and age groups. And to provide ongoing collaboration between the treatment team and case workers. • Have input into and assist care planning for patients. This includes completion of paperwork prior to discharge, addressing aftercare needs and placement plans. • Ensure treatment arrangements. • Document patient and patient family's responsibility to individual and/or family therapy in the client's chart on a routine basis. Document all contact with other agencies and for all sessions with clients. • Document all arrangements made for care planning in the medical record, including completion of Aftercare Plan. • Attend and contribute to treatment planning sessions. Completion of treatment plans for assigned patients for both insurance and COMAR regulations. • Responsible for supervisor signing off on chart documentation accordingly. Essential Functions: • Maintain 25-30 billable service units per week. • Complete documentation in the EMR within 72 hours of completed sessions. • Complete treatment plan developments before the 5th session and ensure signatures by all members of the client's treatment team. • Have a minimum of once monthly family therapy sessions with clients present. • Coordinate referrals for additional levels of service as needed. • Address any and all clinical concerns with a direct supervisor. • Participate in weekly supervision. • Collaborate with service providers to ensure all areas of clients' treatment needs are being addressed. • Utilize evidence based therapeutic framework to assist in overcoming dependencies, adjusting to life and making changes. • Modify treatment activities and approaches as needed in order to comply with changes in client's status. • Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies. • Counsel family members to assist them in understanding, dealing with and supporting clients. • Maintain confidentiality of records relating to client's treatment. The Licensed Clinical Social Worker providescounseling and crisis intervention to patients/clients who have social oremotional problems in a variety of settings across the age span.
Essential Duties and Responsibilities: - Provides necessary social work intervention
- Counsels individuals and families in crisis and help withissues such as poverty, loss and illness
- Assesses patient's psychosocial needs, plans for, evaluatesand modifies interventions to meet the established patient/client goals
- Collaborates with all disciplines to plan and evaluate teamgoals for each patient/client
- Facilitates patient/client and family referrals to communityagencies when appropriate
- Provides financial counseling when necessary to achieve thetreatment goals established by the multi- disciplinary team
- May be required to respond to crisis situations, includingdomestic violence and child abuse
- May be required to provide witness services and evidence incourt cases
- Visits schools to assess academic concerns
- Works with government agencies to help clients apply forMedicare and Medicaid when appropriate
- Arranges home visits to evaluate a patient/client health,home environment and family interaction/support
- Assists patient/family/caregivers in planning for presentand future psychosocial needs
- Observes and reports patient progress and/or changes inpatient status to physician and appropriate multi- disciplinary team members
- Adheres to facility and Amergis policies and procedures at alltimes
Minimum Requirements: - Master's degree in Social Work from a school of social workaccredited by the Council on Social Work Education required
- Active license as an LCSW in state of practice required
- Minimum of one year of social work experience preferred
- Complies with all relevant professional standards ofpractice
- Participation and completion of Amergis' Competency programwhen applicable
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state and local requirements
- Successful completion of new hire training as applicable tojob site
- Understand patient confidentiality and HIPAArequirements
- Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
- Computer proficiency required
