Licensed Clinical Social Worker We are currently looking for a dedicated Social Worker to join our Care Management team. In this position, you will play a crucial role in providing vital social work services to patients and their families. Your main duties will involve conducting assessments, devising treatment plans, offering counseling, and collaborating with interdisciplinary teams to ensure patients receive optimal care. Salary Range: $75,000 - $110,000 Sign On Bonus: $5,000Licensed Clinical Social Worker Responsibilities:
- Conduct interviews with patients and families to evaluate ongoing care needs
- Perform thorough social work assessments for patients referred for social services
- Maintain accurate documentation and adhere to record-keeping standards
- Provide psychosocial insights for patient transfers between units
- Offer supportive counseling, crisis intervention, and grief counseling as required
- Assist patients in adapting to treatment and managing emotional challenges
- Collaborate with patients and interdisciplinary teams to develop and implement treatment plans
- Offer information, referrals, and advocacy for community resources
Licensed Clinical Social Worker Requirements:
- Hold a Master of Social Work Degree from an accredited School of Social Work
- Possess licensure as a Social Worker (LMSW/LCSW) approved by the State Education Department of the University of the State of New York
- Have a minimum of two years of experience in an acute care hospital setting or related environment
- Obtain NYS SCREEN Certification within the initial 3 months of employment
