This position includes facilitating family meetings, working with anticipatory grief and difficult family dynamics, understanding placement options, and appropriate social, financial, and legal services.Job Duties
  • Provides outpatient psycho-social evaluation for advanced illness patient population.
  • Develops plan of intervention, caseload can consist of inpatients and outpatient as well as family members.
  • Documents in the electronic health record
  • Communicates effectively on an interdisciplinary level throughout the continuum of care
  • Participates actively as a team member in meetings and demonstrates a professional working relationship with all levels of staff
  • Regularly acts as a liaison between medical and nursing staff, patients and outside agencies.
  • Attends to the needs of the advanced complex illness population
  • Participates in in-service and staff development activities by attendance, serving on planning committees and or offering topics for pertinent educational programs
  • Accepts personal responsibility for professional growth and development and own work responsibilities
  • Pursues knowledge of palliative care and its benefit to patients and families; attends conferences and seeks out learning; pursues advanced certification in palliative care social work.
  • Communicates appropriately using good interpersonal skills
  • Serves, manages and supports internal and external customers
  • Participates in performance improvement activities
  • Accurately code services using current billing codes as applicable with system/department procedures
