Job Description:
$12,000 Sign On Bonus!Patient Care
  • Provides outpatient counseling and evidence based mental health counseling interventions.
  • Documents services in EHR for billing submission in compliance with standards as provided by the clinic business office.
  • Completes comprehensive social histories including patient's mental health status, preexisting health, mental health and substance abuse problems, strengths and deficits, social role functioning, employment status, relevant cultural and religious factors, social, behavioral, environmental, and financial issues to develop a clinical assessment and treatment plan consistent with the scope of practice of a Licensed Independent Social Worker as defined by the Iowa Social Work Licensure Act.
  • Collaborates with psychiatrist in caring for Behavioral health patients providing assistance and support as needed.
  • Serves as a resource to the staff, providing guidance and direction for social services and mental health related issues and facilitates.
  • Participates in and presents in-services and continuing education activities as appropriate.
  • Participates at meetings and committees as assigned by manager and additionally meetings sponsored by agencies or groups of agencies to increase knowledge of resources and community networking.
Relationship Building & Patient Education
  • Coordinates care with outside sources including Lab, Diagnostic Imaging, Private Health Care Providers, Social Workers, and Public Health.
  • Balances team and individual responsibilities, objective and open to others' views, gives and welcomes feedback, contributes to positive team goals, and puts success of team above own interest..
  • Utilizes effective education techniques for age related care in behavioral health.
  • Collaborates and works effectively with interdisciplinary team members and community health agencies.

  • Practices in accordance with current ADA Clinical Practice Guidelines
  • Collects data, monitors and reports patient outcomes in relation to national benchmarks.
  • Incorporate quality improvement data and/or patient satisfaction data in clinic goals.
  • Assists in unit specific duties (i.e. management responsibilities, as appropriate to the assigned area.
  • Provides education to clinical staff when needed.
  • Attends continuing education programs to maintain licensure.
Basic Clarke County Hospital (CCH) Performance Criteria
  • Demonstrates Clarke County Hospital Standards of Behaviors as well as adheres to hospital policies and procedures and safety guidelines.
  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, DIA, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.
Other Requirements
  • As an employee of Clarke County Family Medicine Clinic, you may be required to assist in response activities for disasters, emergencies and pandemics which may occur outside of normal working hours.
Qualifications:Education: Must possess a Master's Degree in Clinical Social Work and be currently licensed to practice as a Licensed Independent Social Worker in Iowa, or be under supervision to prepare for examination.Experience: Preferred: Previous social services experience in an acute or outpatient setting preferred.License(s)/Certification(s):
  • Valid Driver's license when driving any vehicle for work-related reasons.
  • Current, active, and license in the State of Iowa with appropriate certifications as necessary by licensing boards.
  • MSW required
  • LISW or LMSW required
