Job Title: Licensed Master Social Worker

Location: New York, NY 10065

Shift: 9:00 am - 5:00 pm

Duration: 3 Months with a strong possibility of extension

Pay Range: $55 to $60/Hour.

Duties and Experience:

• Inpatient discharge planning hospital experience.
• Must have Epic and Allscripts experience.
• Hospice, SAR referrals, LTACHs, Acute rehabs, H.D. insurance authorizations, abuse cases, shelter admissions.
• Must carry a case load from 10 to 30 pts independently.

Education:

• Masters in social work.
