Job Title: Licensed Master Social Worker

Location New York 10016

Shift- Mon-Fri, 9 AM to 5 PM

Rate Range - $45 to $50 Per hour

3 Months (Contract) with possible extension

Responsibility:

Mon-Fri. No weekends.

Rare but if needed -holiday/OT possible. Inpatient discharge planning hospital experience. Must have Epic and Allscripts experience. Hospice, SAR referrals, LTACHs, Acute rehabs, H.D. insurance authorizations, abuse cases, shelter admissions.

Can carry a case load from 10 to 30 pts independently.

Masters in SW required. LMSW in NYS is required.
