Responsibilities:
Summary
The Master Social Worker, Licensed position provides an array of reentry and case management related services to participants located within a corrections-setting. This position operates in partnership with the department of corrections within a multivendor treatment model.
Primary Duties and Responsibilities
- The Licensed Master Social Worker provides complex case management services.
- Coordinates discharge planning activities.
- The Licensed Master Social Worker reviews case information, conduct assessments, provides diagnoses (under the supervision of an LCSW-C) and treatment recommendations.
- Assists participants with navigating community resources and services as they reestablish their lives within communities.
- The Licensed Master Social Worker identifies reentry and integration barriers and provides support and assistance in overcoming those barriers.
- Develops, monitors and modifies treatment plan in collaboration with the multi-vendor team.
- The Licensed Master Social Worker serves as a liaison between participants and community resources to ensure efficient processing of referrals (i.e. submit referrals to substance use treatment providers, establish an initial appointment).
- Assists participants with obtaining social services and other healthcare benefits (i.e. health insurance, birth certificates, state identification).
- The Licensed Master Social Worker provides psychoeducation and counseling as it relates to preparation for reentry/transition.
- Conducts individual and group counseling (with focus on substance use treatment).
- The Licensed Master Social Worker consults with colleagues and other DPSCS and GEO staff related to the reentry/transition barriers or challenges.
- Participates in professional meetings, committees, trainings and conferences.
- Participates in research and Quality Assurance and Improvement (QA & I).
- Performs other duties as assigned.
Qualifications:
Minimum Requirements
- Masters degree program from an accredited school of social work.
- Valid/current license as a Licensed Master Social Worker (LMSW) as issued by the state in which the work is performed. Knowledge of the principles, procedures, techniques, trends, and literature of social work with particular reference to clinical social work; psychological or social aspects of mental and developmental and physical disabilities.
- Knowledge of community organization principles; scope and activities of public and private health and welfare agencies; characteristics of mental, developmental, and physical disabilities; current trends in mental health, public health and public welfare, and Federal and State programs in these fields.
- Ability to utilize and effectively apply the required technical knowledge.
- Ability to establish and maintain the confidence and cooperation of persons contacted within the context of work.
- Ability to secure accurate psycho and social data and record such data systematically.
- Ability to prepare clear, accurate, and concise reports.
- Ability to work family and community agencies in preparation for discharge.
- Ability to develop and implement programs.
- Ability to provide professional consultation; analyze situations accurately and take effective action.
- Ability to communicate effectively through written or verbal modes.
- Ability to work with computers and the necessary software typically used by the department.
Pay: Pay USD $74,000.00/Yr. Bonus (if applicable): $2,500.00 Sign-On Bonus!
