Schedule: Monday – Wednesday 10:00 AM - 6:00 PM, Thursday/Friday 11:00 AM - 7:00 PM

SCOPE OF ROLE:

SUS is seeking a passionate and dedicated Social Worker to join our Behavioral Health Programs team. The Social Worker will oversee the delivery and coordination of services, working closely with clinical staff, mental health and medical providers, and community-based organizations to ensure individuals receive optimal care. This role involves conducting psychosocial assessments, supporting individuals in achieving permanent stable housing, and linking them to necessary community services.

The ideal candidate will thrive in a crisis-driven Behavioral Health environment, with expertise in substance abuse, NYC Housing Application (2010E), trauma, and dual diagnosis.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Conduct psychosocial assessments and provide treatment from a strength-based, person-centered perspective.
  • Assess client needs and connect them with appropriate community-based services.
  • Develop treatment plans, identifying problems, strengths, coping skills, and required assistance.
  • Facilitate weekly mental health/substance abuse workshops and onsite support groups.
  • Collaborate with team members to promote client wellness and stability.
  • Maintain knowledge of community resources and make appropriate referrals.
  • Provide consultation on psychosocial needs and the impact of these needs on health care and treatment compliance.
  • Utilize medical and mental health diagnoses and treatment procedures.
  • Manage crisis intervention and stabilization as needed, including occasional after-hours or weekend emergencies.
  • Complete, review, and sign off on all required documentation and reports in a timely manner.
  • Identify program needs and make recommendations for improvement.
  • Maintain compliance with all SUS policies, OMH regulations, and statutes.
  • Perform other tasks as assigned by the program director.

JOB REQUIREMENTS

  • Education & Licensure:
  • LMSW licensure required from a CSWE-accredited school of social work, or LMHC licensure.
  • Experience:
  • Knowledge of emotional/behavioral health, mental health, and dual diagnosis.
  • Skills:
  • Effective written and oral communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Basic computer skills, including proficiency in MS Office.

Preferred Qualifications:

  • Bilingual candidates are preferred but not required.
