The LMSW will provide critical mental health and case management services to patients, including those affected by chronic illness, aging, maternity, and other social challenges. This role offers a $3,500 sign-on bonus (terms and conditions apply).*Key Responsibilities** *Counseling and Crisis Intervention:* Deliver supportive counseling, crisis intervention, and emergency evaluations for patients and their families.* *Intake and Assessment:* Conduct mental health assessments and mental status exams for new patients, coordinating with primary care or psychiatric providers.* *Case Management:* Manage a caseload, providing ongoing case management support and referrals as needed.* *Therapeutic Services:* Facilitate individual, group, and family counseling sessions, supporting integrated care through collaborative group sessions with other providers.* *Resource Coordination:* Connect patients to appropriate community resources and maintain thorough case documentation.* *Compliance and Quality Assurance:* Ensure clinical services align with program grants, funding sources, and clinical standards.* *Patient-Centered Care:* Develop and follow through on treatment plans in collaboration with patients and their families, promoting holistic care based on individual needs.* *Administrative Duties:* Accurately document all clinical interactions, secure necessary service authorizations, and maintain up-to-date patient records.*Qualifications** *Education:* Master's degree in Social Work and CASAC Certification required.* *Licensure:* Active New York State LMSW license required.* *Certifications:* CPR/BLS certification (American Red Cross or American Heart Association, in-person only).* *Experience:* Previous experience in behavioral health, crisis intervention, or case management preferred.*Compensation** *Hourly Rate:* $40.10 - $43.68 per hour, based on experience and licensure* *Benefits Package Includes:** Comprehensive medical, dental, and vision insurance* Life and disability insurance* Retirement savings plan* Flexible spending and health savings accounts* Paid time off, parental leave, and tuition reimbursement* Professional development assistance* Employee assistance program (EAP)* Referral program and signing bonus*Justice, Equity, Diversity, and Inclusion (JEDI) Commitment*Our JEDI initiatives are central to our mission, ensuring that Sun River Health is a welcoming, inclusive environment for all.
