12-week assignment. Mon-Fri. No weekends. Rare but if needed -holiday/OT possible. Interviews via zoom but would prefer to interview in person if the candidate is local. Inpatient hospital discharge planning experience. Epic and Allscripts experience.Hospice, SAR referrals, LTACHs, Acute rehabs, H.D. insurance authorizations, abuse cases, shelter admissions. Can carry a case load from 10 to 30 pts independently. Masters in SW required. LMSW in NYS is required
