Position: Licensed Master Social Worker (LMSW) Location: New York, NY 10038 Shift: 8:30 AM - 4:30 PM (includes a 30-minute break) Department: Care Coordination and Social Work Job Summary: This is a 59-day assignment requiring a Monday to Friday schedule, with possible holiday shifts. The Licensed Master Social Worker will provide vital social work services within a hospital setting, focusing on case management and discharge planning. Responsibilities:
- Conduct comprehensive assessments to determine patients' social and emotional needs.
- Develop and implement effective discharge plans, collaborating with the interdisciplinary care team.
- Manage a caseload efficiently, demonstrating robust multitasking abilities.
- Utilize EPIC EMR for patient management and documentation.
- Handle Allscripts for referral management and coordinate transportation setups as needed.
- Interact and collaborate with various healthcare professionals to ensure comprehensive patient care.
- Support psychiatric and pediatric populations, addressing their unique needs.
Qualifications:
- Master's Degree in Social Work (MSW) (required).
- Licensed Master Social Worker (LMSW) by the State of New York (required).
- Previous hospital social work experience (required).
- Case management experience with a focus on discharge planning (preferred).
- Strong computer navigation skills and familiarity with EPIC EMR and Allscripts (preferred).
