Duties Description As a Licensed Master Social Worker at Capital District Psychiatric Center, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness. Duties include:•Providing crisis intervention, service linkages, and discharge planning•Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed•Determining discharge needs and appropriate community resources•Establishing and maintaining relationships with community providers to ensure continuity of careNOTE:1. In order to be eligible for appointment and to maintain employment, you cannot be listed as an excluded individual or entity on any of the Federal and/or State Medicaid and Medicare exclusion lists (or excluded from any other Federal or Federally assisted program).
