Job Description:

We are hiring a LMHC, LMHC-LP or LMSW to work Full-Time in our outpatient East Harlem office. This position will be hybrid after training is completed.

Position Overview: This position will be filled by a licensed clinical professional, LMSW, LCSW, LMHC, or LMHC-LP, with a degree from a graduate school. They will have extensive experience and training in treatment of individuals with mental illness and co-occurring disorders. The therapist will be assigned recipients who will be treated on an ongoing basis within their caseloads and will develop timely documentation including progress notes, treatment plans, assessments, and any written encounter or collateral contact pertinent to the recipient. The Therapist will attend weekly hourly face-to-face supervision with Program Director or any person deemed appropriate by the Director. Supervision may also include group supervision with peers, task supervision, and trainings for clinical skills development.

Key Functions and Responsibilities:

  • Provide psychotherapy, assessment and treatment planning, discharge planning, crisis intervention, and complex care management. Modalities of service include individual, group, conjoint, and family therapies. Some may be delegated specific tasks i.e., supervision of social work interns or other tasks.
  • Meet with all recipients referred and assess each individual's level of risk and specific needs for assessments. Document all services provided accordingly in the recipient's case chart.
  • In coordination with the medical staff, provide comprehensive services to the outpatient mental health recipients. As recipients are referred, complete and develop a treatment plan. Provide ongoing psychotherapy to the recipients and work as a team with the medical staff, psychiatrist, and other licensed professionals within HELP to ensure all identified needs of recipients met.
  • Identify high-risk recipients and develop individualized treatment plan(s) to address specific needs.
  • Maintain accurate concise records on all recipients of care. Be certain to detail of and progress/lack of progress in psychotherapy notes.
  • Act as a driving force in coordinating services within the facility, bringing to the immediate attention of administration any problems incurred on the recipient's behalf. Act as a representative of the recipient when resolving care difficulties within HELP.

Qualifications:

  • Master's Degree in Social Work or Mental Health Counseling required.
  • Either currently licensed as a LMSW, LCSW, LMHC, or LMHC-LP. As a part of this position, it is expected that the staff member be qualified by training and experience in the treatment of individuals with mental illness and co-occurring disorders.
  • Bi-lingual English/Spanish is preferred.
  • The ability to work independently and compatibly with recipients and co-workers is a must.
  • Self-motivation, organizational, and good communication-skills required.
