JOB DEFINITION:Work with the CN Guidance Youth ACT Team to deliver services to youth who are diagnosed with severe emotional disturbance (SED) and their identified families, whose needs have not been met by more traditional service delivery approaches. EDUCATIONAL REQUIREMENTS:Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist.EXPERIENCE REQUIRED:Two years' experience working with the chronically mentally ill (Masters level). Competency in substance abuse, family support, or vocational/educational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.DUTIES AND RESPONSIBILITIES:Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between Youth ACT and other systems.Provide culturally sensitive and developmentally appropriate treatment to Youth and their families. Services include but are not limited to Assertive Engagement, Case Management, Assessment, Child and Family Services Planning, Individual, group, and/or Family Counseling/therapy, Family Psychoeducation, Individual Psychoeducation, Psychosocial Rehabilitation, Crisis Intervention Services, Vocational/Educational Services. Maintains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations.Provides direct, face-to-face and/or collateral services to recipients either in the Youth ACT Office or in the community in places and situations where problems arise.Must be available (along with other members of the Youth ACT Team) on an as needed basis 24 hours a day, 7 days a week, including responsibility for periodic afterhours coverage on the phone and in person as needed. Assists recipients in developing appropriate expectations from treatment.Strengthens the ability of the recipient and their identified family to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process.Exchanges information with other agencies that benefits the recipient or ensures compliance with regulations and coordination of services. Utilizes and understands the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the Agency. Participate in staff development and providing staff supervision and staff training. Actively engage in agency wide trainings and work groups as well as NYS specific training on and for the Youth ACT model.Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progressComplete QI Training during onboarding and participate in QI refresher training as needed.Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
