As a trusted community partner, WellLife Network fosters a nurturing environment where everyone has the opportunity to thrive and realize their fullest potential.Position Summary: The Licensed Mental Health Professional, (LCSW, LMSW, LMHC) provides comprehensive assessment, diagnosis, treatment planning, treatment (individual and family therapy as indicated), support and education to families with children ages 5 - 18 who are diagnosed with serious emotional disturbance and are at risk of out of home placement (residential placement) or currently living in a residential program or transitioning back to their homes and communities from residential placement The overall programmatic goal is to facilitate family reunification/reintegration by enhancing positive interactions among family members, and, provide families with tools to support stabilization long term in their communities. Essential Accountabilities:Knowledge of childhood psychiatric disorders, trauma, suicidality, developmental tasks, and psychiatric medications. Demonstrates an understanding of common treatment modalities used for childhood psychiatric disorders, particularly evidence based treatments, experience in providing such AND willingness to expand their professional skills through varied trainings. Demonstrate knowledge of the children's mental health system of care. Provide treatment in families' homes and communities establishing dates/times/locations conducive to families' schedules.Conduct initial and ongoing comprehensive assessments to determine strengths, identified needs and to determine efficacy of interventions.Provide ongoing individual and family therapy utilizing age appropriate therapeutic interventions, preferably evidence basedUtilize strategies to integrate cognitive and behavioral changes and emotional self-regulation into the home environment, as identified in the treatment and service plans.Develop and reinforce individualized safety alert plans; provide ongoing crisis prevention services, post crisis debriefing; and explore child and family dynamics related to crisis to prevent further crisis.Partner with the identified child's family and natural supports in supporting treatment and service plan goalsProvide psychosocial education, teach parenting skills, and implement child/family relationship building strategies with family and natural supports. Maintain communication and collaboration with all relevant healthcare and community providers serving child and family. If indicated, provide linkages to and encourage use of community resources that support the youth in their home and community. Demonstrates sensitivity to cultural and ethnic norms, recognizes strengths in culturally driven parenting patterns and approaches to discipline, and demonstrates a non-judgmental approach to working with families whose background and values differ from their ownDemonstrate skills in use of computer to include Avatar, the current EHR. Maintain detailed, timely, and accurate record keeping in the medical record. Participate in individual supervision provided by LCSW to address concerns/issues and improve skill development.Adhere to all OMH and agency policy and procedure such as time keeping, use of vehicles etc.Other duties as assigned.QualificationsLicensed Mental Health Professional (. LCSW, LMHC, LMSW.) AND two years of experience working with children diagnosed with SED and their families. Must have valid and clean NYS driver's license.
