Sapphire Nursing at Meadow Hill , a premier long term skilled nursing facility located in Orange, New York has immediate openings for Full-time LICENSED Physical Therapy Assistant to help in our rehab department! Join our team assisting residents for the best quality of life. Our rehab is very busy clinic looking for a PTA to work with a group of caring staff in sub-acute and long-term care setting. Responsibilities: Physical Therapist will perform evaluations, establish treatment goals and plan for all residents requiring physical therapy services. Screens residents for appropriate equipment, i.e. prosthetic/orthotic appliances, wheelchairs, ambulation aids. Supervise Physical therapy assistants and work with a dynamic team. Skills: • Provides skilled physical therapy services/interventions in accordance with physician orders under the supervision of a Registered Physical Therapist. • Responds to requests for service by relaying information and referral to Registered Physical Therapist. • Implements and modifies the patient's occupational therapy plan of treatment with PT supervision • Monitors patient responses to treatments and modifies it during treatment sessions as necessary. • Observe residents and write progress reports. • The Physical Therapy Assistant will be responsible for documenting patient treatments, changes in condition and other pertinent information as required.
